411 N Central Ave Phoenix AZ 85003. Please go to the 5th floor to suite 550 in the University Center building.

Academic Advising Forms

All document forms should be submited to please include your student ID number

    • Waiver of GPA for department [CRD, TDM, CSM, NLM, PRM] courses
    • Request to change major electives
    • Request for concurrent enrollment of prerequsite 


Allow up to 3 weeks to add/remove certificate
