In person appointments take place at the Downtown Phoenix Campus:

411 N Central Ave Phoenix AZ 85003. Please go to the 5th floor to suite 550 in the University Center building.

Academic Advising Forms

All document forms should be submited to scrdadvising@asu.edu please include your student ID number

    • Waiver of GPA for department [CRD, TDM, CSM, NLM, PRM] courses
    • Request to change major electives
    • Request for concurrent enrollment of prerequsite 

 

Allow up to 3 weeks to add/remove certificate
