PETITION AND REQUEST FORMSForm required to register for an independent study (499) course. Form required to register for an omnibus (580, 583, 584) graduate course. Form required to register for an omnibus (590, 592) graduate course. Petition for change to core requirement, waiver of GPA restriction, acceptance of transfer course, change to course ralted area, waiver of prerequiste or other (explain) Students in the School of Community Resources & Development have the right to file a grievance toward a full-time or part-time faculty member alleging an unfair or arbitrary decision (including grade appeals) in evaluating academic performance or other treatment received in the classroom or educational setting. At ASU, the power to overturn a grade rests at the Dean’s (college) level. However, prior to filing a formal grievance with the college, a student must utilize the informal process at the school level. The College of Public Programs student grievance process can be found at http://copp.asu.edu/students/studentgrievance.html The grade of "I" (Incomplete) can only be given by an instructor when a student, who is doing otherwise acceptable work, is unabile to complete a course (e.g., final exam or term paper) becasue of illness or other conditions beyond the student's control. |
